Psoriasis and vitiligo are some of the most common dermatoses. It is estimated that psoriasis affects up to 3% of the population, while vitiligo affects about 1% of the population. Psoriasis

Psoriasis is a non-infectious systemic skin disease of a chronic and recurrent nature, which can be diagnosed on the basis of characteristic changes on the skin. They are usually papules covered with silvery scales and pustules. They occur primarily around the knees, elbows, cross and scalp. Less commonly, psoriasis appears on the hands, feet, nails or in intimate places. It is the resultant of environmental factors and manifests itself in people who are genetically predisposed to it. Obesity may also be a risk factor for the disease. Psoriasis causes changes not only in smooth skin, but also in joints, scalp and nails. Treating this ailment is easy because its etiology is unknown.

There are, for example:

  • plaque psoriasis (also called psoriasis vulgaris),
  • pustular psoriasis,
  • intertriginous psoriasis,
  • psoriatic erythroderma
  • nail psoriasis.
  • Psoriasis can also increase the risk of other health problems, including:
  • arthritis,
  • depression
  • Crohn’s disease,
  • uveitis.

To avoid exacerbation of symptoms, people with psoriasis should adhere to medical check-ups, take care of regular physical activity and a healthy diet, avoid stimulants and stress.

Among the available treatment methods, it is worth mentioning:

  • biological therapy
  • topical medications
  • systemic drugs
  • phototherapy.
Albinism

Vitiligo, like psoriasis, is a genetic disease of unknown etiology. In a person suffering from this ailment, melanocytes, i.e. skin pigment cells, do not function properly. This results in the appearance of completely discolored spots on it, most often located especially on the hands, face, knees and genitals. Apart from the lack of pigment (melanin), they do not show any other ailments. They don’t itch or flake.

It is still not clear how exactly albinism occurs. In simple terms, however, we can say that, as in the case of psoriasis, both genetic and environmental factors have an impact on the development of the disease. Some theories mention disorders in the functioning of the immune and nervous systems, oxidative stress, infections and physical injuries.

Vitiligo may be accompanied by diseases such as:

  • thyroid diseases,
  • diabetes,
  • alopecia areata,
  • premature graying of hair,
  • depression,
  • sickle cell anemia,
  • Addison’s disease.

Among the available treatment methods, it is worth mentioning:

  • therapy with glucocorticoids, calcineurin inhibitors,
  • therapy with vitamin D derivatives,
  • phototherapy,
  • surgical procedures (e.g. transplantation of cultured melanocytes or epidermal transplantation).
Vitiligo types

Vitiligo is divided into two main types: congenital and acquired. They are related to the moment of onset of the ailment, and each of them has two more subtypes related to the range of symptoms:

  • congenital generalized – in its course there are also changes in the retina resulting in photophobia, usually changes on the skin cover more than 80% of its surface,
  • congenital partial – spots have a limited range of occurrence, usually do not cover the hands and feet,
  • acquired generalized – spots occur on the face and limbs – they usually cover more than 80% of the body surface,
  • acquired partial – spots with a limited range, there may be few of them – e.g. one or two, on different parts of the body.
Treatment of psoriasis and vitiligo

To sum up – one of the factors that soothes the lesions in patients affected by this disease is ultraviolet radiation. Therefore, they are recommended to frequent sunbathing, which inhibits the accelerated development of skin cells and the excessive reaction of the immune system, which affects the formation of lesions. In the autumn and winter period, patients are referred to the so-called phototherapy and photochemotherapy using equipment emitting both UVB and UVA radiation.

Waldmann UV5002 cabin

At the Medical Clinic Podhale Art in Zakopane, we have one of the most modern phototherapy cabins on the market – Waldmann UV5002. This device makes the treatment of psoriasis and vitiligo extremely effective. Allows irradiation of patients with a narrow scope 311 nm UVB (21 lamps) or UVA (21 lamps) rays. It is equipped with:

  • intelligent cooling system, thanks to which the treatment is extremely comfortable,
  • a system of mirrors enabling uniform irradiation of the patient’s body,
  • sensors precisely dosing radiation.

To obtain the best therapeutic effects, it is recommended to irradiate about 2-3 times a week in 15-20 sessions.

PUVA therapy

PUVA (Psolaren Ultra Violet A) is a therapy consisting in administering to the Patient a natural, plant chemical compound – psolaren (found, among others, in celery or lovage). This substance has properties that sensitize the skin to ultraviolet, making the radiation more effective. Irradiation should be repeated 3 times a week. However, the dose of selected UVA rays (320-400 nm) is selected individually, depending on the patient’s skin reactivity. This method is also used in the treatment of vitiligo, atopic dermatitis, lichen planus or photodermatosis.

UVB 311 therapy

This method consists in the selective use of a specific wavelength of UVB radiation (wavelength – 311 nm) with proven therapeutic properties, without the administration of psolaren. The therapeutic effect is achieved by simply exposing the skin to light. This therapy is also used in the treatment of vitiligo and sometimes in the case of atopic dermatitis, pruritus or pruritus.
